1. Spray tanning is accomplished by the application of a solution containing the active ingredient, DHA (Dihydroxyacetone). DHA is considered to be safe and has been FDA approved for cosmetic use.
2. All ingredients in this product used in this procedure are intended for cosmetic use and generally regarded safe. There are, however, occasions where individuals may be allergic to one or more ingredients in the spray tan solution. Liquid Sun Rayz tanning is not responsible for any allergic reactions. We urge you to check with your physician before using our services if you (i) have any respiratory conditions such as asthma, (ii) have any questionable skin allergies or (iii) are pregnant or nursing.
3. There are very small percentage of people whose skin does not favorably react to spray tanning. For this reason we do NOT advise being sprayed for the first time when your appearance is crucial; wedding, photographic session, modeling assignment,etc.
4. Some fabrics such as such as silk and satin may be stained by the spray tanning solution. Please use caution and care . It is best to wear dark loose fitting clothing after the tanning session. Use a towel for leather and light colored seats on warm days and an umbrella for rainy days to minimize rub off.
5. Pregnant or nursing women should consult their physician before being spray tanned.
6. This product does not contain sunscreen and does not protect against windburn.
7. We do not guarantee results, if you have any other tanning products on your skin that have not been showered off , at the time of your appointment with LSR.

I hereby consent to receiving spray tanning services from Liquid Sun Rayz tanning. I hereby waive any and all claims or rights on my behalf as a result of any service rendered and release and forever discharge Liquid Sun Rayz and its employees from any and all claims and responsibility or cause of action arising from the rendering of tanning services and or treatments. To my knowledge, I have no medical conditions or allergies which would preclude me from having this procedure done. I agree to hold Liquid Sun Rayz harmless of any and all medical complications that may arise. I have been advised to discontinue use and consult a physician if any reaction occurs. I agree to hold Liquid Sun Rayz harmless of any and all damage or loss of personal items. I have read and understand this consent form by clicking accept.

Cancellation Policy (For Tanning at a show)

We issue NO REFUNDS. NO EXCEPTIONS. CREDITS(COUPON) will be issued towards future shows MINUS a $50 cancellation fee (THIS IN NON REFUNDABLE IN ANY CASES). Credits expire 1 year from the date of cancellation. Credits and appointment cannot be transferred to another person. No credits will be issued if an appointment is canceled less than 2 weeks prior to a show.

Full payment is required to book an appointment. If payment is not made, your information will expire and you must repeat the booking process.

 

Returns

30 Day Return Policy – Unused Products Only

Returns are only accepted within 30 days of the date of purchase. To be eligible for a return, your item must be unused and in the same condition that you received it. It must also be in the original packaging. 

All items are subject to a 20% restocking fee on all returnable items within the 30 days of the purchasing date. 

To return your product, please contact us at customerservice@liquidsunrayz.com To process your return, we require a receipt or proof of purchase. You will be responsible for shipping costs. If the product is received by us in unused and undamaged condition and in its original packaging, we will refund your purchase less 20% restocking fee. It might take 2 weeks for the returning process.

We are responsible for your new purchase until it reaches the shipping carrier of your choice. After that, it is out of our control and we recommend that you select USPS Priority or Express shipping service. Insurance is to protect against loss or damage during shipping. If you choose not to buy insurance and your purchase is lost or damaged by USPS or UPS, we cannot provide a refund. 

In the case that your shipment is lost or damaged in transit; YOU ARE RESPONSIBLE TO FILE THE CLAIM WITH SHIPPING CARRIER.


 WE ARE NOT RESPONSIBLE FOR LOST PACKAGES DURING TRANSIT!
 WE ARE NOT RESPONSIBLE FOR SHIPMENTS SENT TO FORWARDING SERVICES OR COMPANIES REGARDLESS OF SHIPPING SERVICE SELECTED, IF YOU PLACE AN ORDER AND YOUR SHIPPING ADDRESS IS A FORWARDING ADDRESS YOU ARE RESPONSIBLE FOR CONTACTING YOUR FORWARDING COMPANY OR SERVICE TO LOCATE YOUR SHIPMENT.

WE ARE ALSO NOT RESPONSIBLE TO WEATHER RELATED DAMAGE OR DELAYS! ONCE PACKAGE LEAVES OUR HANDS IT IS COMPLETELY OUT OF OUR CONTROL!



Please make sure your address is correct before placing order we will not be responsible for incorrect addresses. Packages are NOT insured, if you would like to purchase insurance for your package please select USPS Priority or Express shipping service. Once again we are not responsible for package once it transfers to SHIPPING CARRIER, it is then their responsibility to deliver and handle concerns for shipping delays or lost packages. You will not be refunded for the purchase from us if the carrier loses the package unless you purchase the additional insurance. 

If your package is returned you are responsible to contact us with the correct address to be shipped back to and for the shipping service of your choice.

SHIPPING FEES ARE NOT REFUNDABLE: Please make sure all addresses are correct before placing your order, we will not refund shipping fees for wrong address on your part or returned shipments. We will not refund shipping fees on returns initiated by the customer. Only if we sent the wrong product and you decide you don't want a replacement we will refund the shipping fee, we will request the products back or that you destroy the product and provide photographic proof that the product is not longer usable before we process the refund.
